| Puppy Contract

The buyer agrees to obtain from the seller a dog for the amount of: $___________ Less deposit: ___________ Balance: ___________ Paid on & Initials: _______________ Breed: Shetland Sheepdog (Sheltie) Sire: Cody Dam: Patches Sex: Male Female Color: Sable & White Merle Tri Date of birth: 2/25/08 This dog is not being sold under co-ownership terms. Payment in full must be made prior to buyer taking ownership of the dog. Payment is to be made in full as cash only. 30 Day Health Guarantee: The seller certifies that, upon receipt, the dog is in good health. This dog has the following conditions now: ______________________________________________________________________________________ Those conditions are excluded from any guarantee and are not valid to reject the dog due to these conditions. The health guarantee is valid for 30 days from the date of pickup. Health guarantee covers only genetic disorders, birth defects, etc. The buyer agrees to have the dog examined by their own veterinarian at their own expense within 14 days from the date of purchase. If the veterinarian does not deem the dog “fit for sale”, buyer must notify the seller immediately. Seller reserves the right to have the dog re-examined by their veterinarian at their expense. Any condition that is minor, correctable/curable or a breed related minor condition is NOT covered. If the dog is then found not “fit for sale”, buyer has 2 options. They can return the dog to the seller for a refund or replacement dog to be given in it’s place, or they can keep the dog and receive a 50% refund of the purchase price and then assumes all responsibility for care and treatment of the dog- these conditions must be decided and agreed upon by both parties. Seller does not accept responsibility for contagious diseases diagnosed after 14 days from the date of purchase. Conditions such as heat stroke, exhaustion, dehydration and other heat related conditions are not covered in any way. Conditions such as worms, colds, respiratory infections and other such sicknesses are also not covered. ACA registration papers will be given with the dog. However, seller does not guarantee the dog on size, color or potential look of the dog. The dog is being sold as pet quality only and not a show or potential breeding dog. Under no condition will there be a refund of money unless the dog is diagnosed with a life threatening disease or birth/genetic defect. Buyer’s Responsibility: The buyer agrees to take good care of this dog, feed and house the dog properly and maintain appropriate shots. The death, injury or loss of said dog due to theft, carelessness, recklessness, abuse, neglect, heat stroke or accident will not entitle the buyer to any compensation from the seller. No warranty is given to the buyer for any special purpose or function of this dog. Notes: Dog will come with vet check, 1st shot, and first de-worming.
